UGH syndrome is a sight threatening iatrogenic disease most often caused by a poorly positioned IOL. Avoiding UGH is the best solution but prompt diagnosis and intervention can be curative if it occurs. Ultrasound bio-microscopy allows dynamic viewing of the IOL, lens capsule and uvea in suspected UGH syndrome. Out of the bag, pseudo out of the bag and in the bag UGH syndromes are reviewed.
